Six Steps to Success education programme supporting Palliative and End of Life Care


Aim: To increase knowledge, skills, confidence and access to evidence based palliative and end of life care throughout the education programme. The Six Steps to Success programme focusses on care homes and home based care teams, achieving quality improvement outcomes and quality experiences for individuals and families at end of life. The Six Steps to Success education programme is based on six core standardised workshops, advance care planning, anticipatory care planning, quality end of life care and bereavement support within their care settings.

A successful approach to focusing on early assessment, support and coordination for care homes and home based care teams. The programme supports best practice in key Welsh Government documents including Palliative and End of Life Care Delivery Plan (2017), Social Services and Wellbeing (Wales) Act (2014) and Care Inspectorate Wales.


The Six Steps to Success programme was adopted from North West England (NHS) and implemented by the Macmillan End of Life Care Nurse Facilitation Team in Betsi Cadwaladr University Health Board, North Wales, in 2013. In 2020/21, Theresa Richards (Betsi Cadwaladr UHB), Tracey Evans (Wrexham County Borough Council (WCBC), Julia Pugh and Lisa Wooler (Cwm Taf Morgannwg UHB), and Sarah Wheeler (Powys Teaching HB). The team worked together to spread the education programme in their areas in Wales and we hope to see the Six Steps to Success education programme adopted throughout Wales.
